refactor: restructure specs and plans directories

Browse Source 
- Specs (brainstorming output) now go to docs/superpowers/specs/
- Plans (writing-plans output) now go to docs/superpowers/plans/
- User preferences for locations override these defaults
- Update all skill references and test files
