revert: restore phase-based workflows with flexibility

Mode-switching was overcomplicated. Back to phases with explicit permission
to revisit earlier phases when constraints emerge or validation fails.

Simpler, clearer, preserves what worked.

## Overview
Execute implementation plans by switching between reviewing, executing, and handling blockers based on current needs.
Load plan, review critically, execute tasks in batches, report for review between batches.

- Tests fail but you mark task completed
- Unclear instruction but you guess instead of asking
- Partner updating plan but you don't switch to REVIEWING
