Jesse Vincent 03360d1a21 Add end-to-end brainstorm handoff test 
8-turn brainstorming flow that tests whether Claude invokes writing-plans
(correct) or EnterPlanMode (bug) after design approval. Includes
--without-fix mode for failure reproduction attempts.
